We are so pleased that you have decided to renew/join the club! We understand that you might have filled out something like this before but we do require it in full this year. Couples who are joining must also fill out the form for each person. Sorry! You may choose to pay your membership dues directly online right now (preferred) or you can also wait to pay at the Annual Meeting. Please note that any member who has not paid by the evening of the AGM (Dec 03), will have a particularly frosty British response.
